Welcome to the Skyler Cope CPR Required Training Forms page. Before attending any of our courses—BLS, ACLS, PALS, First Aid, or specialized emergency response training—students are required to complete all necessary forms. These forms ensure your safety, confirm your eligibility, and help us provide the best possible training experience.

• Liability Waivers & Release of Claims: Protects you and our instructors while acknowledging the inherent risks of hands-on training.

• Medical and Emergency Information Forms: Helps us respond quickly and appropriately in case of a medical situation during training.

• Photo/Video Consent Forms: Optional forms for those who wish to allow the use of images or videos for educational and promotional purposes.

• Course-Specific Requirements: Any additional documents needed for specialized training.

Please complete all forms before your course date to ensure a smooth check-in process. Forms can be submitted online through Google Forms or downloaded and brought with you to class.
